  • Property & Environmental Hazards: Equipment failures, structural damage, or unsafe conditions within SIL homes or community venues.
  • Behavioral & Emotional Distress: Unexpected crises, distress, or situations requiring emergency intervention or de-escalation.
  • Participant Rights & Safety Concerns: Allegations of neglect, abuse, financial exploitation, unauthorized restrictive practices, or privacy breaches.

We manage all incidents with speed, compassion, and full transparency using our structured 5-step procedure:

  1. Immediate Safety & First Aid: Our staff immediately render first aid, arrange emergency medical care, and secure the environment to protect all individuals involved.
  2. Notification & Reporting: Support workers notify care management right away. Incident reports are logged internally within 24 hours of occurrence.
  3. Reportable Incidents Escalation: If an incident meets NDIS criteria for a Reportable Incident (e.g., serious injury, abuse, neglect, severe medical emergency), we formally notify the NDIS Quality and Safeguards Commission within mandatory statutory timeframes (24 hours to 5 days).
  4. Investigation & Root-Cause Analysis: Our operations team conducts a thorough, confidential investigation to understand why the event happened and identify contributing factors.
  5. Corrective Actions & Continuous Learning: We implement preventative measures—such as updating care plans, repairing equipment, retraining staff, or altering safety protocols—to ensure similar incidents do not reoccur.

We believe in complete transparency. Following any incident involving a participant, our care coordination team contacts the participant, their family, or designated emergency contact promptly to:

  • Confirm the participant’s current safety and wellbeing status.
  • Explain what happened clearly and openly.
  • Outline the immediate steps taken and further investigation plans.
  • Involve the participant and their advocate in determining any adjustments needed in their support plan.
